How can I request a refill on my prescription?
If you require a routine prescription refill, please submit your request directly through your pharmacy at least 10 days in advance.
Please note that prescription requests from pharmacies may take up to 3 business days to process.
How do I book an appointment?
To schedule an appointment, please contact our clinic directly at 416-551-8181.
One of our receptionists will be happy to assist you.
Please note that our clinic can be very busy. If we are unable to answer your call, kindly leave a detailed message including your name, phone number, and reason for your call, and we will return your call as soon as possible.
I have a document or form that needs to be completed by the doctor. How should I proceed?
Please drop off the form during business hours with your portion fully completed.
The doctor will review the form, and you will be notified if an appointment is required or if there is any associated cost before the form is completed.
Please note that form completion may take up to 3 weeks.
