Are you accepting new patients?
How can I request a refill on my prescription?
If you need a refill for your current medication, please contact our clinic directly by phone or email at least 10 days before your medication runs out. This allows us enough time to process your request and ensure your prescription is refilled before your current supply ends.
How do I book an appointment?

Please note that our clinic can be very busy. If we are unable to answer your call, kindly leave a detailed message including your name, phone number, and reason for your call, and we will return your call as soon as possible.
I have a document or form that needs to be completed by the doctor. How should I proceed?
Please drop off the form during business hours with your portion fully completed.
The doctor will review the form, and you will be notified if an appointment is required or if there is any associated cost before the form is completed.
Please note that form completion may take up to 3 weeks.
